Cardiac Health Services
A Three-Phase Program of Care
Rehabilitation is a key component of heart care at Cardiac Health Services. We offer a three-tiered program, aimed at rehabilitating heart patients who need assistance in adapting to lifestyle changes and improving their cardiovascular fitness.
Phase One is usually administered on an inpatient basis, after the patient has already experienced heart problems. This phase includes medical care, education, exercise and emotional support. Diagnostic services may also be a part of this phase, but are based on one's own needs. At this time, many patients usually need to look at their risk factors and determine if any lifestyle changes are needed. Phase One provides the counseling and education so critical for increasing cardiovascular health. The approach, however, is based solely on individual need.
Phase
Phase Two consists of a 12-week monitored exercise program, which includes nutrition counseling, stress management classes and continued education. This part of the program is designed for people with known heart disease or who have had a heart attack or heart-related events such as surgery or PTCA. Phase Two may begin within one week of a person's discharge from the hospital. Patients initially work one-to-one with a health care professional on exercise and education. In the beginning an integral component of Phase Two is family involvement, especially if lifestyle changes are warranted. Upon graduating from Phase Two, patients join the Phase Three supervised program. This allows continued exercise along with the educational support of staff.
Phase Three of Cardiac Health Services’s services is a supervised exercise program which individuals learn to improve their overall cardiovascular health. It is designed for people with known risk factors for those with existing heart problems or for those simply wishing to improve their level of cardiovascular fitness. The initial visit entails assessment by Cardiac Health Services staff. Each person’s medical needs are evaluated and a program is customized according to individual need.
Phases One and Two need physician referral. Phase Three allows for physician referral or self-referral.
