Dobutamine Stress Echocardiogram (DSE)

What is a Dobutamine Stress Test?

• This is a test that can be done on patients that cannot exercise on a treadmill.
• Dobutamine is a medication that increases heart rate and contractility, similar to the effects of exercise.
• This helps the doctor determine if the heart is getting an adequate supply of blood and oxygen when it is working harder.
• The procedure includes an echocardiogram done at rest, and again at peak heart rate. This procedure is a noninvasive test that uses ultrasound (sound waves) to evaluate the structures of the heart, and how well the heart is working.
• As the patient is positioned on their left side, warmed gel is applied to the chest and a wand – like device called a transducer is moved over the chest to produce images of the heart and it’s structures.
• Electrodes are placed on the chest to record EKGs before, during, and after the test. This records the heart rate and rhythm.
• A registered nurse starts an IV through which the dobutamine is given.
• A cardiologist is present for this test and observes the patient for any symptoms, as well as their EKG, blood pressure, and echo images.
• Preparation and the actual test take about 90 minutes.
• This test must be ordered by a doctor and be scheduled.

Why is a Dobutamine Stress Echo done?

• This test will help the doctor evaluate the patient’s cardiac status related to:
• If the patient has had any heart attacks in the past.
• If any symptoms develop and at what heart rate.
• Irregular heart rhythms.
• If there is an adequate blood and oxygen supply to the heart at rest, and when it is working harder.
• The size, appearance, and efficiency of the heart’s valves and chambers.

What Preparation is Needed?

• Do not apply ointments, lotions, or powder to the chest. Deodorant can be worn.
• Wear a comfortable, loose, two-piece outfit.
• Do not smoke, eat, or drink anything except water for four hours before test.
• It is important to ask your doctor :
• If Beta Blockers should be stopped 72 hours (three days) before the test.
• If regular medications should be taken the day of the test.
• How to adjust food, insulin, and diabetes medication prior to the test.

Registration 

• Registration will occur in the Cardiac Health Services department. You will need to bring a picture ID (such as a driver’s license), insurance cards, and any paperwork from the doctor. Bring a list of the medications you take.
• Plan to arrive 60 minutes before the test to allow time for preparation.
• Enter the hospital through the Outpatient Building.
• Take the elevators to Cardiac Health Services department, which is located on the second floor.
